CPE EVENT INITIAL NOTIFICATION

If you are holding an event for which you will award CPE under the COPAS CPE licensing you must first notify us of the event with the form. Once you have notified us you will then provide required details using the CPE EVENT NOTIFICATION DETAILS form.

  • Name of group holding event
  • MM slash DD slash YYYY The date the event will be held
  • This is required for the allocation of a class number. Enter the name of the presentation here
  • The time the event will start INCLUDE THE TIME ZONE
  • The event location is a physical address for live in person classes and a platform for online classes (Adobe Connect/Zoom/MSTeams/GoToMeeting etc.)
  • This is the person who is the contact for the COPAS office who will take care of all the administration paperwork/tasks for the event
  • This is the email address that the COPAS Office will use for contact regarding CPE for the event
  • This is the number at which the COPAS Office can reach the Event Contact
